The 12-point checklist
- Itemized bill in hand.
- Pharmacy receipts attached.
- Implant invoice (if applicable).
- Original discharge summary (not a photocopy).
- All lab and imaging reports.
- Follow-up appointment scheduled in writing.
- Medication list with generic alternatives noted.
- Dietary and activity restrictions documented.
- Insurance final settlement amount.
- Any pending payments clearly listed.
- Contact number for post-discharge queries.
- Patient feedback form completed (yes, it matters).
